What Blood Tests Should Women Get in Their 40s?
Your 40s are a pivotal decade for health. Several conditions that quietly develop during these years—high cholesterol and elevated blood sugar—have no symptoms early on but are easy to detect with a simple blood draw and very treatable when caught early. Here is a plain-language guide to the blood tests that matter most in your 40s, based on national guidelines.
The Blood Tests Backed by Strong Evidence
Cholesterol (Lipid Panel)
A lipid panel measures total cholesterol, LDL ("bad") cholesterol, HDL ("good") cholesterol, and triglycerides. Cardiovascular disease is the leading cause of death in women, and cholesterol tends to rise during the perimenopausal years, making your 40s an important time to check it.
The American College of Cardiology and American Heart Association suggest checking a lipid panel every 4 to 6 years for adults ages 20 to 75, and more often if you have risk factors.
The U.S. Preventive Services Task Force recommends lipid testing as part of cardiovascular risk assessment in adults ages 40 to 75.
You do not need to fast beforehand—a non-fasting sample is accurate for estimating your risk.
Your cholesterol numbers, along with your blood pressure, smoking status, and other factors, are used to calculate your 10-year heart disease risk, which helps you and your clinician decide whether a cholesterol-lowering medication (statin) would help.
Blood Sugar (Diabetes and Prediabetes Screening)
Type 2 diabetes often causes no symptoms for years, yet damage to blood vessels and organs can begin during the prediabetes stage. Screening is done with a hemoglobin A1C, a fasting glucose, or a glucose tolerance test.
The USPSTF recommends screening adults ages 35 to 70 who are overweight or obese, repeating every 3 years if normal.
The American Diabetes Association recommends screening all adults starting at age 35.
Screening earlier or more often is reasonable if you have a family history of diabetes, had gestational diabetes during a pregnancy, or have other risk factors.
Key thresholds: an A1C of 5.7% to 6.4% indicates prediabetes; 6.5% or higher indicates diabetes.
Optional Blood Tests to Refine Your Heart Risk
Beyond the standard lipid panel, a few specialized blood tests can help clarify your cardiovascular risk, especially if you have a family history of early heart disease, a personal history of heart or vascular disease, or a borderline/intermediate risk estimate. These are options to discuss with your clinician rather than universal requirements.
Lipoprotein(a), or "Lp(a)"
Lp(a) is an inherited type of cholesterol particle that raises the risk of heart attack and aortic valve disease. Major heart societies (including the American Heart Association/American College of Cardiology, the European Society of Cardiology/European Atherosclerosis Society, and the National Lipid Association) now recommend measuring Lp(a) at least once in every adult's lifetime.
Because levels are about 90% determined by genetics, a single measurement usually suffices—it does not need to be repeated.
A level above roughly 50 mg/dL (or 125 nmol/L) signals higher cardiovascular risk.
Lp(a) can rise after menopause, so if your level was borderline before menopause, a repeat check afterward is reasonable.
A high Lp(a) does not yet have a specific approved medication, but it is a signal to manage all other risk factors more aggressively, including lowering LDL cholesterol.
High-Sensitivity C-Reactive Protein (hs-CRP)
Hs-CRP measures low-grade inflammation in blood vessels and can add information about heart risk, particularly for women whose standard risk factors look normal.
Levels below 1, 1 to 3, and above 3 mg/L correspond to lower, average, and higher relative cardiovascular risk.
A value above 10 mg/L usually reflects a temporary infection and should be rechecked in 2 to 3 weeks; testing is most meaningful when you are well.
Historically this test was reserved for those at intermediate risk, though some newer expert statements favor broader use.
Other inflammatory markers (such as fibrinogen or interleukin-6) generally add little beyond hs-CRP and are not recommended for routine use.
What About Estrogen and Other Hormones?
Many women in their 40s wonder whether they should have their estrogen or other hormone levels checked to find out if they are in perimenopause. For most women, the answer is no—routine hormone testing is not recommended and is not needed to diagnose the menopause transition.
Perimenopause is diagnosed by your symptoms, not a blood test. In healthy women over 45, the diagnosis is based on your age, changes in your menstrual cycle, and symptoms such as hot flashes—laboratory tests are not required.
Hormone levels swing too much to be reliable. Estrogen (estradiol) and follicle-stimulating hormone (FSH) rise and fall dramatically from day to day and cycle to cycle during perimenopause. A single result can look "normal," "premenopausal," or "postmenopausal" depending on the day it is drawn, so it rarely changes what you and your clinician do. The Choosing Wisely campaign specifically advises against checking FSH in women in their 40s to identify the menopause transition.
FSH and estradiol are also not used to decide on or adjust the dose of menopausal hormone therapy.
When hormone testing does make sense:
Symptoms before age 40 (or roughly 40 to 45). If periods stop or become irregular with menopausal symptoms at an unusually young age, FSH and estradiol help evaluate for premature or early ovarian insufficiency. This usually warrants a referral.
Ruling out other causes. If symptoms are atypical, a thyroid test (TSH) and a pregnancy test are often more useful than sex-hormone levels, since thyroid disease can mimic menopausal symptoms and perimenopausal women can still become pregnant.
Fertility questions. If you are trying to conceive or considering it, tests of ovarian reserve—such as anti-Müllerian hormone (AMH), along with FSH and estradiol—may be used, but AMH is not a reliable way to diagnose menopause or predict its exact timing.
Tests Based on Your Individual Situation
Depending on your history and risk factors, your clinician may also recommend:
Sexually transmitted infection testing (such as gonorrhea, chlamydia, or syphilis), based on your age and risk factors.
Thyroid testing (TSH) if you have symptoms such as fatigue, weight changes, or menstrual changes. Routine thyroid testing is not recommended for women without symptoms.
Additional tests related to specific personal or family medical history.
Beyond Blood Tests: Don't Forget These in Your 40s
Some of the most important screenings in your 40s are not blood tests:
Blood pressure: checked at least every 1 to 2 years, or annually starting at age 40.
Mammogram (breast cancer screening): The USPSTF now recommends all women begin mammograms every other year starting at age 40.
Cervical cancer screening (Pap and/or HPV test): continue on schedule, typically every 3 to 5 years.
Colon cancer screening: begins at age 45 for average-risk adults.
Depression screening and counseling on tobacco, alcohol, diet, and physical activity.
The Bottom Line
For most women in their 40s, the core blood tests are a cholesterol (lipid) panel and a blood sugar test if you are overweight/obese or have other risk factors. Specialized heart-risk tests such as lipoprotein(a) and hs-CRP can further clarify your cardiovascular risk. Talk with your clinician about your personal and family history—it shapes which tests are right for you and how often you need them.
